Process

  • 1 Submit the application via: Website: Electronic Provider Services - Central Laboratory Portal
  • 2 set a date for site visit for testing.
  • 3 Internal procedures, the application is processed in case the transaction is accepted and fee paid electronically.
  • 4 Receive the testing report electronically.
